We’re looking forward to your dog’s stay with us!

If your dog is staying with us for the first time, please complete the information below so we have all the information in advance of your dog’s stay with us! While most fields are not required to allow returning clients to only update pertinent information, the more thorough your answers, the better we will be positioned to work with your dog!

If you are are returning client, please visit your dog’s webpage for your most recent updates to this profile and complete any fields that require updates, corrections, or new information.

Your contract will be sent separately with the cost breakdown specific to your stay.

Contract and policy agreements will be provided as follows:

I understand that by agreeing to a payment plan I have committed to the full length of the training program as stated in the Description of Services above. I understand that I am responsible for timely making payment in full at the start of services, regardless of whether I choose to complete the training program. If any payments are not timely made, Sit ‘n Stay Training is not obligated to provide further services.  

Follow up sessions may be scheduled (two follow up sessions for stays over 1 week are included as a complementary service) within 3 months of completion of stay - these may be scheduled virtually, in-person, or in-home dependent upon client preference and trainer discretion.  

Charlotte will endeavor to create as safe an environment as possible for the training of my dog and will offer only sound, safe, and responsible training and training instructions. However, to the extent that Charlotte Hoberg is insured for any unintentional or negligent errors, omissions, or incorrect assertions, Charlotte Hoberg will be responsible for any such acts or omissions, but only to the extent of such insurance.

I have been told by Charlotte Hoberg and understand the inherent risks of owning a dog, including but not limited to the risk of dog bites to myself or others. I represent and warrant that I have provided Charlotte Hoberg with full and complete, accurate information regarding any bite history and similar incidents or hazardous tendencies of my dog, and that I will update that information if it changes. I understand that I am and will remain responsible for the actions of my dog at all times, and I hereby agree to indemnify, release, and hold harmless Charlotte Hoberg of any and all claims, whether made my myself or any third party, of injury, expense, costs, or damages caused by my dog. I understand that the recommendation of any other product or service is not a guarantee of my satisfaction with that product or service.

This contract, together with language expressly incorporated into it in writing, is the full and complete agreement between myself and Charlotte Hoberg. A complete and accurate copy of this contract is as valid as the original. This contract is made valid by in-person signatures, electronically signed signatures, or upon receipt of a signed, scanned copy by email.

I authorize Charlotte Hoberg to take my dog into public spaces during his/her stay for the purpose of training my dog. Dependent upon training goals and scheduling, this may include travel by car across state lines in order to provide appropriate decompression hikes, training experiences, and novel exposure including but not limited to public parks, public pet-friendly locations, private dwellings, trial and competition environments, training club, and group training classes or outings.

I authorize emergency medical care to be provided for my dog(s) by the above-named veterinarian, or an appropriate alternate to be determined by Charlotte Hoberg in the event that my regular veterinarian is not available or that closer care is required.  I will immediately reimburse Charlotte Hoberg for any charges related to emergency care, including office visits, procedures, medications, surgeries, etc.

Cancellation Policy: A deposit equal to 25% of the full balance of the stay is due upon booking and confirmation. This deposit is non-refundable. For the remaining balance, please be advised that cancellations made up to 72 hours before a scheduled appointment via email, text, or phone call will be processed without a penalty. Cancellations made 72 hours or less before an appointment may be subject to a charge of the full service rate.
